    Tim Sweeney (Epic).
    John Carmack (Id).
    DJ Delorie (Delorie).

I'm sure if more programmers had the access that Carmack had
(as in getting cards/new versions of APIs months before their
release), they'd be able to do some amazing things, too.

    As for worst professional programmer:

Dan Gookin. I think that's his name. I don't like "C for Dummies."

I'm with ethic here. I still think hes a damn fine programmer. Anyone whos seem some of what Doom III is bringing has got to bow to the man. Hes been an innovator in the gaming industry (specifically FPS) for years. He basically started to whole concept. Doom and Wolfenstein brought that genre into existance.

John Carmack. I think he's one of the few (maybe only)programmers who comes out with software that pushes the hardware community to catch up. And successfully too.

I know I had to upgrade my PC countless times in order to play his games at decent framerates.
    Try not.

    Demi-Gods don't count.

My vote would go to many, but Alexander Stepanov comes most readily to mind at the moment. I respect someone who can pick a project and stick with it for 20+ years.

He is attributed to the design and implementation of the C++ STL.
