A really big worm got ........ed that it's species has been put down and used as bait for so long that it decided to take matters into his own hands (so to speak). Alien devices made him explode. He got really mad and had the world's worst aneurysm. Hand of God. The booth fell over and he died. It was time for it to be replaced and a forklift with cylindrical forks picked up the booth and pinned him inside, killing him. The man had a heart attack in the fishyard, and the blood was from the fishes. Any of those right?

I was just trying to point out there's a hundred different ways it could have happened. Whatever the answer is usually isn't the only option in a puzzle like this, because key details are left out (like the truck being refrigerated or there being holes in the telephone booth). Every answer I've given is possible, although improbable, with the criteria set forth.
