Your format string specifies 12 identifiers, but you supply 13 values.
Yeah I posted with haste... I also mistyped a placeholder as well. I was trying to get this post deleted but I couldn't get ahold of a mod. Sorry bout that.
Heh. Np. Next time just edit and point out you discovered your error if a friendly neighborhood mod is unavailable (or unwilling) to come to your aid.
