Here's a demo I wrote a few years ago (100% assembly code). The timing works correctly only in pure DOS, as it uses low level techniques (it directly accesses the PC timer) sorry.

I don't get it. As a note, I was running it on WinXP, despite the warning...but I can appreciate the amount of work that it takes to do anything in ASM, and for that reason...impressive.
